Home and Community-Based Services Waiver
The Department of Human Services has licensed Fellowship Home Care to provide Home and Community-Based Services (HCBS) to people over age 65 and people with disabilities. Under the Minnesota Statutes, Chapter 245D, HCBS-licensed providers may render two forms of support: basic support and intensive support.
Distinct from the level of assistance they provide, basic support focuses on performing personal care and direct supervision of the medical and health-related needs of individuals. Intensive support focuses on promoting the training and rehabilitation of a person. It also covers services directed to the growth of professional home care providers.

With the help of Direct Support Providers (DSP), eligible people over age 65 and people with disabilities may now conveniently seek assistance at Fellowship Home Care. By incorporating government initiatives into our home care delivery, we hope that we may be able to help more people in the community regain independence without being burned by the costs.
